Suspension Submission Form

  • Your suspension will begin 31 days from the submission of this form.

  • Suspensions must be for a minimum of 3 months at a time and requested through this suspension form 31 days prior to the 1st of the desired month you wish to begin your suspension (the 1st of the month is when your membership payment is processed). Any request made less than 31 days in advance of the 1st of that month will result in the following month's payment going through, regardless of your attendance.

  • If your suspension is due to a medical-related circumstance, please email us a signed doctor’s note at and we will suspend your membership immediately.

  • If you are the paying member of a family plan, please include all names of those you wish to suspend in this form.

  • If any of your membership payments have declined, you must first make those payments before your suspension can take effect.

  • Late notices will not be refunded.

  • If your membership has been suspended for more than 6 months, you will not be grandfathered into our original prices.
